Double Eyelid Surgery + Rhinoplasty: Understanding a Combined Approach

When multiple facial features are being considered for surgery, the goal is not simply to change each feature individually, but to evaluate how they relate to the overall facial balance.

In this case at Merry Plastic Surgery, the procedures included double eyelid surgery, rhinoplasty, septal cartilage use, fat grafting, and under-eye fat grafting, performed by Dr. Sungtaek Hong.

What do these procedures address?

Double Eyelid Surgery: Creates or defines an upper-eyelid crease while considering the patient’s existing eyelid structure, skin, and eye shape. Rhinoplasty: Can address the shape and proportions of the nose. Depending on the individual anatomy, structural support may also be considered. Septal Cartilage: Cartilage from the nasal septum can be used as a structural graft material in rhinoplasty when appropriate. Facial Fat Grafting: Fat may be placed in selected areas to improve volume balance and soften areas that appear hollow or uneven. Under-Eye Fat Grafting: Carefully placed fat can be considered for under-eye hollowness, although the suitability and amount required vary significantly between patients. Why is individualized planning important?

The eyelids, nose, cheeks, and under-eye area all contribute to the overall appearance of the face. Changes in one area can influence how another area is perceived. For this reason, a surgical plan should be based on the patient’s anatomy, facial proportions, skin condition, and functional considerations rather than applying the same technique to everyone.

Before & After

The image shows a preoperative and postoperative comparison. Before-and-after photographs can help illustrate changes, but they should not be interpreted as a guarantee of a particular result for another patient. Healing, swelling, tissue characteristics, and surgical planning can all affect the final outcome.

From a clinical perspective, the most important part of combined facial surgery is appropriate patient evaluation and selecting procedures that are medically and aesthetically suitable for that individual.
