Revision rhinoplasty can be more complex than primary nose surgery, particularly when the nose has become noticeably upturned after a previous procedure.

At Merry Plastic Surgery, we approach revision cases by first assessing the existing nasal structure, the amount and condition of available cartilage, skin thickness, scar tissue, and the changes from previous surgery.

In cases where the nose is overly upturned, the goal is not simply to make it “less turned up.” The surgeon may need to restore structural support and carefully adjust the nasal tip and columella to create a more balanced relationship between the nose and the rest of the face.

Why Rib Cartilage May Be Used

When septal or ear cartilage is insufficient because of previous surgery, autologous rib cartilage can be considered as a structural graft material. Because it comes from the patient’s own body, it can provide additional cartilage for rebuilding or reinforcing the nasal framework.

However, rib cartilage is not necessary for every revision rhinoplasty. The appropriate graft material depends on the individual anatomy and the extent of structural changes required.

What This Case Shows

The accompanying before-and-after photographs show a revision rhinoplasty case involving correction of an upturned nasal appearance using autologous rib cartilage.

The 3-month postoperative image provides an early view of the result. It is important to remember that nasal tissues continue to settle and remodel after surgery, so the appearance at three months may not represent the final outcome.
